License renewal is done at the personal request of a member of the Chamber.
The request for renewal of the license is submitted to the Chamber or through the competent office of the Chamber, in person or by mail, no later than 60 days before the expiration of the license.
Documentation required for license issuance:
License renewal request (download the request) with the accompanying documentation specified in the request:
- Proof of payment of the license renewal fee
- Employer certificate (download certificate)
- Evidence - confirmation of the conducted procedure of continuous education
- Certificate of passing the license exam (if a member of the Chamber took the license exam)
- Consent to make a card (download consent)
NOTE:
Evidence - a certificate of the conducted procedure of continuous education is issued by the office of the Chamber to which the member belongs, based on data from the electronic database and is attached to the submitted documentation. If not all acquired points are recorded in the electronic database of the Chamber, it is necessary to submit the original or a certified photocopy of the certificate / s as proof that the acquired points have not been recorded.
Reporting changes: If there have been changes in terms of personal, professional, etc. data after entry in the Directory of members of the Chamber, along with the above documentation you need to submit form EV-2 (download the form).
